Working as part of a national programme, NHS Newcastle Gateshead Clinical Commissioning Group’s (CCG) Pathway of Care Team has been shortlisted for the national Nursing Times ‘Team of the Year’ award for their work with older people living in care homes in Newcastle and Gateshead.
The team is made up of a range of health, social care and voluntary sector professionals, including nurses, GPs, pharmacists, social care workers and care home staff from across nine local organisations. It works closely with local care homes to improve the health and wellbeing of residents.
Lesley Bainbridge, Lead Nurse of the Care Home Vanguard Programme at NHS Newcastle Gateshead Clinical Commissioning Group (CCG) said “The team has worked incredibly hard to influence the best possible care for older people living in care homes, so it is an honour to be shortlisted for this award.
“Older people living in care homes are among the most vulnerable in our society, and their needs are so complex that meeting them can be hugely challenging.
“An ageing population combined with limited resources adds to these challenges, so it is important that we make these proactive changes to not only make a difference within local care homes, but also in the wider health and social care system.”
The winners of the Nursing Times Awards will be announced at a ceremony held on 2 November at the Grosvenor House Hotel, London.
